Instructions to upload test results
Please complete this COVID Test Results Form by uploading a photo or scanned copy confirming negative coronavirus test results from an approved Sars-CoV-2/COVID-19 test. For more information, please see this FAQ in Google Docs.
Please provide a scanned PDF, JPEG/image file, or screenshot of test results, which should include the following:
- Date of test
- Name of testing agency/lab
- Confirmation of an approved Sars-CoV-2/COVID-19 test
- Confirmation of test results
There are several methods you can use to submit a confirmation of a negative test result. Take a photo or screenshot of the results and attach the photo to the form, or upload a PDF of the results.
If your test results come as an email attachment, save the email attachment to your phone or computer and then upload the file to the form.
